We do not ask for medical histories, diagnoses, health conditions, dates of birth, or insurance identification numbers. We also collect standard, non-identifying analytics about site usage.
How we use and share it
We use your information to connect you with a vetted dental provider partner so they can follow up with a written estimate, and to operate and improve the site. When you submit the form and give consent, we share the details you provided with a dental provider partner. That partner pays us for the introduction. We do not sell your information to unrelated third parties for their own marketing.

By checking the consent box and submitting the form, you agree that we and a dental provider partner may contact you by phone, text message (including autodialed or prerecorded messages), and email at the number and address you provide. Consent is not a condition of any purchase. Message and data rates may apply. You can opt out of texts by replying STOP and can ask us to delete your information at any time using the contact method below.
Your choices and rights
